Lake City Public Schools
iPad Usage and Protection Plan Agreement

Each student that is issued an iPad is responsible for his/her own device at all times.
Annual Protection Plan Premium (Optional): $40 per student/year with a $100 family maximum.

Replacement Coverage:
● The District’s protection plan coverage is voluntary.
● The District protection plan provides protection for accidental damage and theft, not lost devices. A police
report must accompany a stolen device.
● The District has sole authority to determine any and all replacement costs and fees due to damage, theft, and
lost devices. Families are responsible to pay for any replacement costs/fees before the student is permitted to
bring the device home.
● If damage/theft does occur, the District reserves the right to require students to check in/out the device daily.
● Damaged devices must be returned to the School District before replacement can be determined.
Limits:
● Coverage shall be limited to two claims per year or up to $299 in total. All repair/replacement costs thereafter
are the full responsibility of the students/parents/guardians.
● iPad chargers/cords are not a part of this agreement. Students will be charged for lost or damaged chargers.
● iPad cases are not a part of this agreement. Under normal wear and tear, the iPad cases should last for three
years. Students will be charged for damaged cases.
